Acute Kidney Injury in Acute Thrombolytic Therapy: The Hunter New England AngioJet Experience

Purpose: The purpose of this study is to ascertain the incidence of acute kidney injury (AKI) in patients undergoing AngioJet thrombectomy for acute arterial thromboses, and to investigate risk factors for acute kidney injury in this patient cohort. Methodology: Retrospective single tertiary centre observational study for consecutive patients who received thrombolytic therapy for acute limb ischemia over the last 10 years between 2015 and 2024. Patients who underwent AngioJet thrombectomy were compared to patients who underwent catheter directed lysis to evaluate the incidence of AKI associated with both techniques. Of the patients submitted to AngioJet thrombectomy, comparisons were made between patients with and without acute kidney injury to investigate risk factors for AKI. Eligibility criteria included patients aged 18 and older, acute arterial thromboses treated with endovascular intervention, serum creatinine measurements performed pre-operatively and within 72 hours post-operatively. Exclusion criteria included patients on haemodialysis or peritoneal dialysis and contraindication to anticoagulation or thrombolysis. Primary outcome defined as greater or equal to 1.5 fold increase in serum creatinine or risk to greater or equal to 26.5mmol/L within 72 hours post-operatively. Results: 309 consecutive patients who required arterial thrombolytic therapy at John Hunter Hospital demonstrated a mean creatinine rise compared to baseline creatinine levels in the AngioJet thrombectomy population. This mean creatinine rise increased in patients who underwent common iliac and external iliac artery AngioJet thrombectomy. Conclusion: AngioJet thrombectomy was not associated with increased risk of AKI, but larger clot burden was associated with increased risk of AKI.
