Purpose Blunt traumatic aortic injury (BTAI) is a life-threatening condition caused by high-impact trauma, with mortality rates often higher in regional settings due to delayed care (1). Endovascular aortic repair has become the preferred treatment for selected injuries. Methodology A single centre retrospective audit of patients diagnosed with BTAI from 2010-25 was conducted in a regional vascular centre. Patients had CT confirmed BTAI and subsequently underwent aortic stenting. Results Of the 20 patients (85% male), 70% of patients required interhospital transfer. 52% of injuries were from a motor vehicle accident, 17% from motor bike accident, and the remainder being falls. The BTAI severity was predominantly Grade 3 (17 patients, 85%) based on the Society of Vascular Surgery grading system. GORE CTAG was used in 12 cases, Medtronic Valiant in 3 cases, Cook Alpha and Endurant in 2 cases each (in one case data not available). All cases were performed via percutaneous femoral access, with 55% cases requiring additional upper limb access. Left subclavian revascularisation was performed in 5 (25%) of cases with 2 (10%) left carotid-subclavian bypass and 3 (15%) subclavian stents. Mean operating time was 71 minutes. Accurate deployment was defined as adequate proximal and distal seal zones with no further intervention, which was accomplished in 95% of cases. 2 patients were noted to have endoleak intraoperatively for which 1 case required angioplasty. No conversions to open surgery were required, and no intraoperative deaths, strokes, or spinal ischemia occurred. Less than 50% of patients underwent long term follow up, and of available data, no graft complications were noted.
