Purpose: Describe a novel assisted technique for technically difficult embolectomy without over-the-wire variants of embolectomy catheter.
Background: The Fogarty embolectomy catheter is the de facto gold standard for management of acute limb ischemia secondary to thromboembolic disease. Current embolectomy catheter options are designed for over-the-wire, arterial, venous, and bypass graft use and universally require passage distal to the embolus for successful extraction. This passage may become difficult due to eccentric plaque, which deforms the relatively flexible embolectomy catheter and prevents distal passage through the embolus. Over-the-wire embolectomy catheters are unaffected and are associated with improved vessel clearance, but are more costly than standard embolectomy catheters and may be scarce.
Methodology: The technique involves the following;
1. Standard technique of open access to the femoral artery, distal and proximal control and insertion of a wire passing distal to the embolus
2. Insertion of dilator, followed by insertion of large sheath (7Fr) over the wire, passing distal to the embolus
3. Dilatator removed, insertion of embolectomy catheter, running parallel to wire within sheath and tip passing distal to embolus and sheath, with balloon clear of sheath
5. While the wire remains stationary, the embolectomy catheter is inflated and retracted, with both the embolectomy balloon and the sheath moving proximally
6. If required, repeat process again as necessary (insertion of large sheath, insertion of embolectomy catheter parallel to wire inside sheath, inflation of embolectomy catheter, withdrawal of embolectomy catheter with wire remaining in situ).
Results: This improvised technique was previously used successfully in 4 cases complicated by the difficult passage of the embolectomy catheter.
Conclusion: A novel assisted technique with standard endovascular equipment improves the success of embolectomy when an over-the-wire catheter is unavailable.
