Purpose
Digital amputations are frequently used in the treatment of diabetic foot ulcers and infections. It is widely believed that hallux amputations disrupt gait patterns and foot anatomy, leading to poor outcomes compared to lesser toe amputations. However, there is minimal evidence to support this idea from research comparing the short and long-term outcomes of patients undergoing hallux versus lesser toe amputations. Additionally, there are only a few studies comparing outcomes of transphalangeal with transmetatarsal amputations. The aim of our study was to compare the following outcomes; ipsilateral further digital amputations, major amputations, major adverse cardiovascular events (MACE), and death.
Methodology
A retrospective cohort study was conducted amongst vascular surgery inpatients of Cairns Hospital from January 2015 to December 2024. Patients with a diagnosis of diabetes mellitus undergoing single hallux or lesser digit amputation were included. Those undergoing digital amputation for pathology other than diabetic foot disease were excluded. Outcomes were examined using Cox-proportional hazard analyses.
Results
Of the 480 patients that were included, 207 underwent hallux amputation and 273 underwent a lesser toe amputation. A significant proportion of hallux amputations were performed as an emergency procedure (p = 0.014). There was no significant difference between the proportion of patients requiring further digital amputation or ipsilateral major limb amputation. Time to an ipsilateral further digital amputation was significantly shorter in the hallux amputation group (p = 0.021). There was no significant difference between incidence of MACE or post-operative death between the two groups.
Conclusion
Minor diabetic foot amputations are associated with risk of repeat digital amputation, MALE, MACE, death. There are comparable rates of these consequences amongst those undergoing hallux or lesser toe amputation; one is not “safer” than another.
