Background Chronic wounds impose a significant clinical burden by increasing morbidity, healthcare costs, and reducing quality of life. Blue light photobiomodulation has emerged as a promising adjunct in wound care, stimulating cellular metabolism, angiogenesis, and antimicrobial action. The EmoLED photobiomodulation device, which has been evaluated in several trials, has intrigued multiple Australian providers. Objectives To evaluate the efficacy of the EmoLED device in aiding the healing of static wounds in vascular and high-risk foot patients. Methods This study is a single-centre, prospective observational study conducted at Sunshine Coast University Hospital. Eligible participants include patients with non-healing wounds under vascular surgery or high-risk foot services. The 12-week study includes a 4-week run-in phase (standard care), 4-week intervention phase (weekly EmoLED plus standard care), and 4-week follow-up phase (standard care). Patients were reviewed 3-months and 6-months post-trial to document wound progress. Healing was assessed weekly through photography and planimetry. The primary outcome is percentage surface area reduction while secondary outcomes include subjective pain and quality of life scores. Results Analysis of 20 participants has been performed. Variable outcomes were achieved during initial phases. Several previously deteriorating wounds showed improvement, whilst others worsened or showed minimal change. The most significant healing occurred 1 to 6 months post-therapy, suggesting a delayed response to treatment. Pain scores consistently improved throughout treatment. Conclusion EmoLED therapy demonstrates variable outcomes during early treatment phases, with possible long-term benefits evident. While most wounds improved, no consistent advantage over standard care was observed in this short-term study. Future studies with longer observation periods may reveal more significant benefits.
