Purpose/Aim
Transcarotid artery revascularisation (TCAR) is an established technique for carotid revascularisation in the USA, utilising a proprietary neuroprotection and stent system which, until recently, has not been available overseas. Select centres in Australia have adopted an “off-the-shelf approach” to establish flow reversal using existing non-proprietary equipment with satisfactory results. As one of the centres historically performing off-the-shelf TCAR, we sought to evaluate our TCAR results following the availability of the ENROUTE Neuroprotection and Stent Systems in Australia.
Methods
We conducted a retrospective analysis of all patients undergoing TCAR with the ENROUTE Neuroprotection and Stent Systems (Boston Scientific) at our institution from March to May 2026. Patients were selected for TCAR where they were considered high surgical risk under historical Centers for Medicare and Medicaid Services (CMS) carotid stent criteria, where they had risk factors making either CEA or TFCAS higher risk, or where there was clinical equipoise between techniques. The primary outcomes were the 30-day ipsilateral stroke rate and 30-day composite stroke/death rate.
Results
10 patients in total underwent TCAR with the ENROUTE system during this period.
7 patients (70%) were symptomatic and 3 (30%) were asymptomatic. 5 lesions (50%) were high grade (80-99% stenosis). 7 patients received clopidogrel (70%) and 3 patients received ticagrelor (30%) as the P2Y12 inhibitor for their pre-operative and post-operative antiplatelet regimen. 3 patients (30%) were standard risk and 7 patients (70%) were high risk according to CMS carotid stent criteria. Technical success rate was 100%. The flow reversal time during the procedures was 10.8 minutes. The ipsilateral stroke rate at 30 days and composite stroke/death rate at 30 days were both 0%.
Conclusion
Our early experience with the ENROUTE Neuroprotection and Stent Systems suggests that it is a safe means of performing TCAR in Australia.
