Purpose: Informed consent requires equitable health literacy. Standard vascular consent forms induce cognitive fatigue through excessive length and medical jargon, disenfranchising lower socioeconomic and culturally diverse patients. While artificial intelligence simplifies text, its ability to translate medicolegal risks without critical omission remains a literature gap. This study evaluates next-generation AI (ChatGPT-5.5) in democratising surgical consent, balancing accessibility with strict medicolegal safety. Methodology: Institutional consent texts for ten major vascular procedures (including EVAR and amputation) were sourced. Baseline word counts, jargon density, and readability (Flesch-Kincaid Grade Level [FKGL]) were calculated. Texts were processed by ChatGPT-5.5, prompted to enhance health literacy to an eighth-grade target. AI outputs were reassessed. Vascular clinicians graded AI outputs (1-5 scale) to ensure severe medicolegal risks (e.g., mortality, limb loss, stroke) were rigorously retained. Results: Baseline texts were highly inefficient (mean 1,678 words) and saturated with jargon, presenting a severe cognitive burden. ChatGPT-5.5 drastically improved accessibility, reducing the mean word count by 74% (to 435 words) and eliminating 88.8% of complex terminology. Readability was further optimised from a baseline FKGL of 5.75 to 5.10. Crucially, clinical grading confirmed 100% of severe medicolegal risks were safely preserved in the AI translation (Mean Clinical Safety Score 5.0/5). Conclusion: Verbose, jargon-heavy institutional consent protocols systemically disadvantage vulnerable populations, risking uninformed consent and medicolegal exposure. ChatGPT-5.5 provides a safe, highly efficient solution, reducing patient cognitive load by 74% while maintaining perfect medicolegal integrity. Clinicians in high-acuity environments can utilise this tool to generate accessible, patient-specific consent documents, fostering true shared decision-making.
