Introduction: Chronic venous disease is a common condition and has a major impact on patients and health resources. The management of superficial venous incompetence has been subjected to level 1 evidence. The aim of this study was to report the contemporary management of superficial venous incompetence within the VASCUNET network. Methods: Vascunet registries were contacted and those that capture data on superficial venous disease were invited to participate. Patients treated with open surgery or endovenous therapy (radiofrequency/laser) between 2016-2022 were included and the data was harmonised. Eight countries (Australia, New Zealand, Finland, Germany, France, Malta, Sweden, Switzerland) contributed to the dataset. The outcomes of interest included variation in treatment options over time, proportion of CEAP 6 and anaesthesia use. Results: 127,047 patients were included and the mean (standard deviation) age was 54.9 (14.6) and 65.8% were females. The proportion of endovenous therapy (compared to open surgery) ranged from 18-100% of cases. The most common endovenous therapy use was radiofrequency, followed by laser then glue. The great saphenous vein was the most common location of surgery/intervention, with proportions ranging between 60% and 80% for all countries. The proportion of general anaesthetic use reported ranged from 9.9% to 84.3%. Of those patients with CEAP 6 New Zealand had the highest reported group with 12.6% compared to France with 0.8%. There was an overall decline in open surgery with most of the change seen in Australia. There were no changes in the reported procedures performed during the pandemic (2020-2021). Conclusion: There are major variations in the management of patients with superficial venous disease across countries and such data might help and understand this variation and standardise patient care.
