Purpose: Splenic artery aneurysms (SAAs) are the most common visceral artery aneurysm and carry a risk of rupture associated with significant morbidity and mortality. While open surgical repair has traditionally been the standard treatment, endovascular techniques, including stent-assisted coil embolisation, have emerged as less invasive alternatives. Experience with stent-assisted coil embolisation is well described in management of intracerebral aneurysms but remains limited in visceral artery aneurysms. This study aimed to review contemporary management strategies for SAAs and present our institutional experience with stent-assisted coil embolisation.
Methodology: A review was performed using MEDLINE and Embase for studies reporting outcomes following open and endovascular management of SAAs. Demographic, procedural and outcome data were extracted. In parallel, a retrospective review was undertaken of patients treated with stent-assisted coil embolisation for SAAs at our institution.
Results: A total of 21 studies comprising 150 patients were included. Management strategies included open repair, coil embolisation, covered stent placement and stent-assisted coil embolisation. Endovascular intervention demonstrated high technical success and low peri-procedural morbidity. In our case series (n=3), stent-assisted coil embolisation achieved aneurysm exclusion while preserving splenic artery patency. Technical success was achieved in all cases without peri-procedural mortality or splenic infarction. During a median follow-up of 3 months, complete aneurysm exclusion was maintained in all patients without reintervention.
Conclusion: Contemporary management of SAAs is increasingly endovascular, with treatment tailored to aneurysm morphology and patient factors. Stent-assisted coil embolisation appears to provide durable aneurysm exclusion while preserving splenic perfusion in selected patients. Our experience supports this as a safe and effective treatment for selected SAAs
